Prolonged Transient Global Amnesia: Part of the Clinical Spectrum or a Separate Disease Entity?

Background Transient global amnesia (TGA) is the prototypical neurologic disease for acute-onset reversible amnesia. It currently defined by resolution of symptoms within 24-hours. In this case report we describe an atypical prolonged TGA, emphasizing our current lack knowledge surrounding entity and its pathophysiology. Results A 66-year old female presented acutely with profound anterograde variable retrograde no inciting event. thorough workup to exclude alternative causes (including computed tomography angiogram electroencephalogram) was normal. Her magnetic resonance imaging consistent punctate diffusion restriction changes bilaterally in hippocampi. She also mildly hypoxemic discernible cause. ultimately diagnosed TGA although her diagnosis remains controversial as persisted 72-hours. Conclusion Our patients clinical features (apart from protracted time-course hypoxemia) were keeping a TGA. The association hypoxemia, COVID-19, obstructive sleep apnea, development be elucidated. Although underlying pathophysiology unknown several mechanisms have been postulated including cortical spreading depression hypoxic-ischemic injury. time course symptom resolution, could important clue discerning on individual basis. Importantly, clinician should not deterred amnestic lasting >24-hours, if clinical/radiologic presentation
